§ 11-63. Same—By corporation.

In the case of a corporation, the application under this division shall contain the following information:
(1)

The names of the principal officers and the name of the person who is actually to perform such services for the corporation, together with the business and home addresses of each of such persons;

(2)

The description of the equipment to be used in such removal, transportation and disposal; and

(3)

The exact location of and the method of disposal.
